At the risk of sounding like a broken record, I am pasting this directly from the official rules for merchants:

Therefore, merchants cannot refuse to complete a purchase transaction because a cardholder refuses to provide ID. Visa believes merchants should not ask for ID as part of their regular card acceptance procedures. (Emphasis is Visa's) Laws in several states also make it illegal for merchants to write a cardholder’s personal information, such as an address or phone number, on a sales receipt.
In other words, you can ask for an ID but if the customer refuses, you must accept the card anyhow. The only exception is if the card is not signed or says "SEE ID" in the signature panel. In that case, you MUST ask for an ID and the customer MUST sign the card or you may NOT accept the card. The words “Not Valid Without Signature” appear above, below, or beside the signature panel on all Visa cards.
